John Kennedy won the final at Rio de Janeiro's Maracana stadium for Fluminense with a 99th-minute volley, only to be sent off for excessive celebrations. Boca Juniors also ended the game with 10 players as Frank Fabra was dismissed.
Rio-based Fluminense had led from German Cano in the 36th minute but Buenos Aires side Boca Juniors were level in the 72nd when Luis Advincula was on target from the edge of the penalty area.
Fluminense had never before won the most important South American club trophy in their 121-year history. They lost the 2008 final to Ecuador's Quito.
